A new drug is created. It goes through animal testing, then, if safe, it goes through testing in humans to find the proper dosage and to see how it should be used and not used. This information is critically evaluated and summarized in a package insert distributed with the drug. Wouldn’t it be great if drugs of abuse went through this process? Unfortunately, they do not. What we have instead are bits and pieces of information, often based on adverse effects or outright disasters that occurred with uncontrolled use of these substances. The healthcare provider is thrust into this minefield of unknowns when encountering a pregnant or breastfeeding patient who is using a substance of abuse. Some clinicians may refuse to deal with these women, while others rely on limited personal experience or snippets of information picked up from others during their careers. Frank Nice and his colleagues have addressed this lack of information for those who deal with pregnant and nursing mothers with substance abuse issues. This book brings the medication expertise of pharmacists together with the expertise of experienced clinicians of other disciplines. It sets down guidelines from professional organizations into one place to rationally assess the information that is available, while putting them into an organized and useable context. Case vignettes sprinkled throughout the chapters provide a sense of the “real world” that the reader may encounter in practice. No book of this nature can ever be entirely complete or cover every possible situation because of the uncontrolled nature of substance abuse and lack of high-quality information. Nevertheless, the authors have done well in creating a rational framework that can guide clinical decision-making. This book should serve well as a starting point for inexperienced clinicians and a solid resource for more experienced practitioners who may not have the formal pharmacologic background that the authors bring to the discussion.

It has been almost 20 years since my first trip to Haiti. I had just presented at a conference on the use of medication during breastfeeding and was attending a dinner that the conference sponsors were hosting for the speakers. Approximately eight of us were enjoying our food when one of the attendees brought up the topic of “White privilege.” She is a Black woman who had spoken at the conference on breastfeeding in the Black population in the United States. My ears perked up, and I thought immediately about growing up in a poor coal mining town in Pennsylvania where my two Polish-born grandfathers had worked the coal mines and ended up dying from the effects of Black (ironic terminology) Lung Disease. My father was fortunate enough to drive a coal truck with his sixth-grade education and only had lingering effects of Black Lung Disease. He would later die from Parkinson’s disease instead. Because of the work ethic shown by my grandparents and my parents, I was the first in my extended family to go to college to get a degree. I earned a BS in Pharmacy. My thoughts were, “What ‘White privilege’ did my family and I ‘inherit?’” Those thoughts ground to a halt when the woman then stated: “If you say you are not a racist, you are a racist.” Therefore, I had two obvious responses to this declaration: 1. I am a racist (affirmed by statement of fact). 2. I am not a racist (affirmed by inference). Of course, no matter what response I gave, I would condemn myself to being a racist. I then pondered that there had to be another response, thinking what it would be. 1. I have a lot of Black friends. Then, I thought that whenever someone is accused of being a racist, this is the first statement one makes in defense. It is usually dismissed because it is possible to have many Black friends and still be racist. Not adequate “proof.” 2. I am married to a Korean woman. This was better, but still not enough “proof.” Even though my wife is a “woman of color” and a “minority,” she is not a Black woman. I could still be racist when it comes to Blacks. 3. One of my daughters is married to a Black man, and I have three Black grandchildren. Once again, this is probably not sufficient “proof.” My daughter made the choice to marry a Black man; it was not my choice. It had to be my choice. 4. It was then that I thought about my medical missions to Haiti over the past 20 years. I did not have to “say” that I was not a racist to affirm that I was, in fact, a racist, by the Black woman speaker’s inference. It would be my body, mind, and soul actions that would affirm that I was not, in fact, a racist. In addition, if that were possible for one person, it was then possible for others. Growing up in a typical middle-class family in the small coal-mining town of Plymouth, Pennsylvania, I only knew of one Black family that had ever lived in my town. I did not meet another Black individual until I went to a special summer course at Bucknell University in Lewisburg, Pennsylvania for students who had excelled in high school. It was not until one year later when I attended Temple University in Philadelphia that I became immersed in the Black community. Never in my whole life, basically spent in that small coal-mining town, did I ever think I would end up ever going to a country such as Haiti. As close in miles and air time as is the poorest country in the world to the richest country in the world, so little is known about what it is like to live in Haiti. It is because of the challenge of the Black speaker at the breastfeeding conference and because of the encouragement of my friends that I write this book. Haiti, indeed, is a country beyond belief, but it is also a country that is not beyond faith in God and beyond hope in their fellow human beings, whatever their race or color.
