The Matrix Intensive Outpatient Treatment for People with Stimulant Use Disorders, Counselor's Treatment Manual, originally published in 1995 by the Matrix Center and reproduced by Samhsa in 2006, has been used by thousands of addiction treatment counselors. The Matrix treatment model grew from a need for structured, evidence-based treatment for clients who abuse or are dependent on stimulant drugs, particularly methamphetamine and cocaine. The program model has been well received by the addiction treatment field, and the demand for the Matrix program materials has been very high. At the same time, as the field became more experienced with the use of the program materials, a need was identified for tailoring of program materials to certain special populations. In 2005, an adaptation to the Matrix materials was developed for Native American/ American Indian persons. In 2006, an additional adaptation was prepared for men who have sex with men.

R esearchers are only beginning to realize the pervasiveness of substance abuse among people age 60 and older. Until relatively recently, alcohol and prescription drug misuse, which affects as many as 17 percent of older adults, was not discussed in either the substance abuse or the gerontological literature. The reasons for this silence are varied: Health care providers tend to overlook substance abuse and misuse among older people, mistaking the symptoms for those of dementia, depression, or other problems common to older adults. In addition, older adults are more likely to hide their substance abuse and less likely to seek professional help. Many relatives of older individuals with substance use disorders, particularly their adult children, are ashamed of the problem and choose not to address it. The result is thousands of older adults who need treatment and do not receive it.

This Tip, Substance Abuse Treatment and Family Therapy, addresses how substance abuse affects the entire family and how substance abuse treatment providers can use principles from family therapy to change the interactions among family members. The Tip provides basic information about family therapy for substance abuse treatment professionals, and basic information about substance abuse treatment for family therapists. The Tip presents the models, techniques, and principles of family therapy, with special attention to the stages of motivation as well as to treatment and recovery. Discussion also focuses on clinical decision making and training, supervision, cultural considerations, special populations, funding, and research. The Tip further identifies future directions for both reasearch and clinical practice.

This Treatment Improvement Protocol (Tip), Clinical Guidelines for the Use of Buprenorphine in the Treatment of Opioid Addiction, provides consensus- and evidence-based treatment guidance for the use of buprenorphine, a new option for the treatment of opioid addiction. The goal of this Tip is to provide physicians with information they can use to make practical and informed decisions about the use of buprenorphine to treat opioid addiction. These guidelines address the pharmacology and physiology of opioids, opioid addiction, and treatment with buprenorphine; describe patient assessment and the choice of opioid addiction treatment options; provide detailed treatment protocols for opioid withdrawal and maintenance therapy with buprenorphine; and include information on the treatment of special populations, e.g., pregnant women, adolescents, and polysubstance users. Clinical Guidelines for the Use of Buprenorphine in the Treatment of Opioid Addiction: Treatment Improvement Protocol Series (TIP 40)

This inservice training manual provides counselors and other clinical staff members with scripted modules to use in trainings for Treatment Improvement Protocol (Tip) 41, Substance Abuse Treatment: Group Therapy, published by the Substance Abuse and Mental Health Services Administration's (Samhsa's) Center for Substance Abuse Treatment (Csat). The seven training modules will assist program staff in understanding and implementing the evidence-based practices described in Tip 41. A Tip is the end result of careful consideration of relevant research findings and experiences in clinical settings. For each Tip, a panel of expert clinical researchers, clinical providers, and program administrators (the consensus panel) discusses the issues relevant to the specific Tip. The product of the panel represents the combined and collaborative input of the various viewpoints and provides recommendations for specific best-practice guidelines.

The revised Tip provides information about new developments in the rapidly growing field of co-occurring substance use and mental disorders and captures the state-ofthe- art in the treatment of people with co-occurring disorders. The Tip focuses on what the substance abuse treatment clinician needs to know and provides that information in an accessible manner. The Tip synthesizes knowledge and grounds it in the practical realities of clinical cases and real situations so the reader will come away with increased knowledge, encouragement, and resourcefulness in working with clients with co-occurring disorders.

This Tip, Medication-Assisted Treatment for Opioid Addiction in Opioid Treatment Programs, incorporates the many changes in medicationassisted treatment for opioid addiction (Mat) that have occurred over the most active decade of change since the inception of this treatment modality approximately 40 years ago. The Tip describes the nature and dimensions of opioid use disorders and their treatment in the United States, including basic principles of Mat and historical and regulatory developments. It presents consensus panel recommendations and evidence-based best practices for treatment of opioid addiction in opioid treatment programs (Otps). It also examines related medical, psychiatric, sociological, and substance use disorders and their treatment as part of a comprehensive maintenance treatment program. The Tip includes a discussion of the ethical considerations that arise in most Otps, and it provides a useful summary of areas for emphasis in successfully administering Mat in Otps.
