The diversity of backgrounds and types of preparation can be a strength, provided there is a common foundation from which counselors work. Clinical supervision itself is much discussed in the professional literature of several disciplines, including social work, psychology, and both mental health and substance use disorder counseling. Theories, process dynamics, and tools abound, but the functional responsibilities and essential skills of the substance use disorder treatment supervisor had not been synthesized into one document that could serve as a standard for the field. To remedy that situation, Csat convened the Clinical Supervision Competencies Task Force (the Task Force) in fall 2005. The Task Force, composed of experts in substance abuse treatment and clinical supervision from across the country, was charged with developing this document, Tap 21-A, Competencies for Substance Abuse Treatment Clinical Supervisors. Tap 21-A is designed to accompany Tap 21, taking monitoring, assessment, and improvement of addiction counseling competencies to the next level.

Competencies for Substance Abuse Treatment Clinical Supervisors (TAP 21-A)

This Technical Assistance Publication (TAP), Clinical Drug Testing in Primary Care, is for clinical practitioners- physicians, nurse practitioners, and physician assistants- in health centers. The publication provides information that practitioners need when deciding whether to introduce drug testing in their practices and gives guidance on implementing drug testing. The TAP does not address drug testing for law enforcement or legal purposes, nor does it include testing for the use of anabolic steroids or performance-enhancing substances. This TAP describes some of the ways that drug testing can contribute to the assessment, diagnosis, and treatment of patients seen in primary care, the management of the treatment of chronic pain and the identification and treatment of substance use disorders.

The abuse of alcohol and other drugs (AOD) is undeniably linked with economic and personal adversities for both individuals and society. It is estimated that the annual national cost of substance abuse is more than $144 billion. This includes related health and mental health care, social welfare, victim's losses, unemployment and lost productivity, and criminal justice system costs. The immeasurable human suffering caused by chemical dependency is equally disturbing. Family dysfunction and violence, children affected by alcohol or other drugs before birth, homelessness and poverty, accidents, homicides, suicides, and crime are often rooted in the abuse of alcohol and other drugs.

The regulations that protect the identities of persons in alcohol or drug abuse treatment have their genesis in two statutes of the early 1970's: the Comprehensive Alcohol Abuse and Alcoholism Prevention, Treatment and Rehabilitation Act of 1970 and the Drug Abuse Prevention, Treatment and Rehabilitation Act of 1972. These statutes were implemented by regulations issued by the then Department of Health, Education and Welfare (HEW) in 1975. Revised in 1987 by one of HEW's successors, the Department of Health and Human Services, the regulations are set out at title 42, part 2, of the Code of Federal Regulations. Recently, Congress reaffirmed and reorganized the original confidentiality statutes by merging them into one act, the Public Health Service Act, now title 42, section 290dd-3, of the United States Code. The need for medical care has been a constant since the day the first merchant ship sailed centuries ago. Concern for the health of merchant mariners has, from the beginning, been a part of our nation's history. In the 1700's, legislation mandated that a Medicine Chest be carried on each American Flag vessel of more than 150 tons, provided it had a crew of ten or more. By 1798, a loose network of marine hospitals, mainly in port cities, was established by Congress to care for sick and disabled American merchant seamen. Called the Marine Hospital Service, later the Public Health and Marine Hospital Service, and finally the Public Health Service, these federal entities continued to provide healthcare to merchant seamen until 1981. The Ship's Medicine Chest and Medical Aid at Sea has been a part of much of this maritime history. This edition has evolved through many previous editions. The Public Health Service published the first Medicine Chest in 1881 under the title, Handbook for the Ship's Medicine Chest.
